After you complete the form, you’ll receive a free 20-minute phone consultation to ensure our services are a good fit for your needs. Expect a call within 1-2 weeks. Appointment wait times are currently at 1-2 weeks.

At the end of the pre-enrollment form, you’ll be asked to provide your payment details. However, your card will not be charged unless you decide after the free consultation that you want to move forward with our services. After your enrollment is confirmed, you will receive a welcome email which includes a link to additional forms that will need to be signed before your appointment. This includes a privacy policy and consent to treat. If you have not gotten this, please check your junk email.

Medicaid Patients

I want to let you know that our practice is currently unable to accept Medicaid. Kentucky law does not allow individuals who are enrolled in Medicaid to pay out of pocket for care or to receive free services from a private-pay medical practice, which means I’m legally unable to see Medicaid recipients at this time.

I understand this may be frustrating, and I’m very sorry. This is not a reflection of your needs or worthiness of care — it’s a legal limitation combined with the reality that our practice is too small to support the staffing requirements involved in working with insurance. If anything changes in the future, I will update this space.
